Introducción
La enfermedad traumática es causa principal de muerte en todo el mundo. Genera 5 millones de muertes/año. Ciento ochenta y dos millones (12%) de años de vida ajustados en función de la discapacidad perdidos por la enfermedad traumática. Quinientos años de pérdida de productividad anual por cada 100.000 habitantes. Solo por accidente de tráfico hay 1,2 millones de muertos/año y 20 – 50 millones de lesionados1.

Centros de trauma
Los CT representan un papel clave y fundamental en los sistemas de atención al trauma. Proporcionan liderazgo, dirección, seguridad y control de la calidad. Es fundamental su capacidad para entrenar en Advanced Trauma Life Support, capacidades, protocolos y adherencias a guías. En general, son hospitales más grandes, universitarios y con más especialidades.
Se categorizan según su nivel de complejidad. Los centros con mayores recursos están capacitados para tratar a los pacientes más críticos.
Conclusiones
Parece que los CT disminuyen la mortalidad en trauma grave o complejo en aquellas regiones en que están bien implantados. Esta reducción puede llegar a ser del 15 – 20%. Obviamente la calidad de la evidencia científica es débil, no existen estudios clase i y escasamente clase ii, aunque el volumen de estudios de evidencia débil es considerable. Mejoran los resultados en enfermedades relevantes, como trauma craneal grave, lesiones medulares, lesiones penetrantes, lesiones en niños, etc.
