A 71-year-old male subjected to clipping of a left anterior cerebral artery aneurysm, adopting an ipsilateral frontoparietal approach. Upon arrival in the Intensive Care Unit, ocular exploration revealed the presence of a gradually increasing accumulation of air at conjunctival level in the left eye (Fig. 1).
On commenting the case with Neurosurgery, the original cause was seen to be opening of the left frontal sinus, with secondary subconjunctival emphysema. Ophthalmology decided conservative management with artificial tears. The normal conjunctiva was restored after 18 days.
