A 32-year-old male was admitted due to attempted suicide after using a rope to hang himself from a tree. Exploration revealed neck lesions and clinical manifestations consistent with incomplete level C2 spinal cord injury. The cervical CAT scan showed a bilateral displaced fracture of the C2 pars interarticularis, extending to laminas (Fig. 1A), hyoid bone fracture and fracture of the left anterior thyroid lamina with airway aperture to the visceral space of the neck, associated with extensive emphysema (Fig. 1B). The MRI study of the cervical spine showed C1–C4 spinal cord contusion and a small intramedullary hemorrhagic focus in the C2–C3 interspace (Fig. 1C).
